Facility Information
When and How to Set Up Facilities Correctly
The Facility section tells insurance companies where services were performed.
This information prints in Box 32 of the CMS-1500 and is required for many claim types.
Important:
In most cases, you should set up at least one Facility, even if it represents the provider’s primary office.
Facilities define the service location that prints in Box 32 of the CMS-1500.
If a provider works at multiple locations, you can assign a default facility and change it at the claim level when needed.
When You Need a Facility
In most cases, you should set up at least one Facility — even if it’s the provider’s main office.
Facilities represent the service location that prints in Box 32 (CMS-1500).
Set up additional facilities if the provider performs services at multiple locations, such as:
- Hospitals
- Nursing homes / assisted living
- Clinics or satellite offices
- Telehealth service locations (when required by payer rules)
Tip: Set the provider’s most common location as the default facility, then change it on the claim when services were performed elsewhere.
How to Access Facility Setup
From the top navigation menu:
- Select Setup
- Choose Facility
- Click Add to create a new facility
If services are performed in the provider’s own office, this section may not be required.
What to Enter
- Facility Information
- Facility Name
- NPI (if applicable)
- State License / Commercial / Location Numbers (only if required)
- Place of Service (critical for claim accuracy)
- Address, City, State, Zip
- Phone / Fax (optional)
Institutional Facility (Checkbox)
Check Institutional Facility only if:
You are billing UB-04 claims
Leave unchecked for standard CMS-1500 claims.
Pay-To Address (Optional)
Only complete the Pay-To Address section if:
Payments should be sent somewhere different than the facility address
If the pay-to address is the same, leave this section blank.
Common Mistakes to Avoid
- Don’t create facilities you don’t need
- Don’t guess at Place of Service codes
- Don’t use facility setup to replace provider information
